Course/Lecture/Session Registration Form

I am of 18 years or older and am participating in the above seminar, lecture, certification, or session sponsored by the Northeast Holistic Center of my own free will. I understand that any or all certifications, classes or lectures that I participate in for certification or to receive credit for, must be completed to the satisfaction of the teacher and the director of the NHC. This includes full (on-time) attendance of the class for the assigned period of time, tests, examinations, practicum work and homework.

RESERVATIONS: Class, certification, lecture reservations or consultations (phone or in person) must be paid in full and received one week in advance of said date.  Last minute enrollments in classes have an additional fee.

CANCELLATIONS: More than one week in advance are charged a $25.00 processing fee for an individual class or session. If the registration is for a series of classes or a complete certification, the cancellation must be done no later than the second class in the series or certification – a $50.00 processing fee for series classes or certifications applies. No other cancellations or refunds are allowed if less than seven days prior to the scheduled activity.

IN CASE OF EMERGENCIES: Fees are held and class or sessions are rescheduled.
